Description

Adhesion has become a common language that connects all specialties within contemporary dentistry. 
However, few clinical relationships demand as much precision, coordination, and clarity as the one between Endodontics and Oral Rehabilitation. Today, we face the challenge of making decisions that can no longer be addressed in isolation: each root canal treatment must be planned in accordance with the type of final restoration, the amount of remaining tooth structure, the condition of the exposed dentin substrate, and the long-term goals of the restorative treatment.

In response to this clinical need for integration, concepts such as the Endo-Resto approach have emerged, aiming to organize and structure post-endodontic decisions through predictable adhesive protocols. This lecture offers both a critical reflection and a practical guide to effectively integrating both disciplines. The first step is to recognize the importance of teamwork: the endodontist and the restorative dentist must operate under a shared vision, focused on preserving as much dental structure as possible while ensuring the long-term viability of the tooth.

Next, we will explore the current principles of direct adhesion to the dentin complex, especially in endodontically treated teeth. We will discuss the selection of light-cured and dual-cured adhesive systems, their polymerization mechanisms, compatibility with resin cements, and their role in achieving Immediate Adhesive Sealing (IAS)
Additional emphasis will be placed on the influence of endodontic irrigation protocols on adhesion, the management of exposed collagen, control of residual moisture, and the inhibition of matrix metalloproteinases (MMPs) - all critical factors for durable bonding.

The third part of the lecture will focus on step-by-step clinical protocols that ensure a smooth transition between endodontic obturation and the definitive restoration. Clinical sequences will be presented, ranging from the use of modern core build-up materials to the judicious and often-debated indication for adhesive posts when necessary - always prioritizing structural reinforcement and marginal integrity.

This integrated approach not only improves the long-term prognosis of the treated tooth but also elevates the standard of care, fostering a modern, conservative, and adhesion-centered dentistry. When all specialties communicate using a shared restorative logic, outcomes become more predictable, and the ultimate beneficiary is the patient.

We hope this lecture contributes to strengthening interspecialty synergy, breaking down barriers between endodontic therapy and rehabilitation, and promoting a more coordinated, evidence-based, and excellence-driven clinical practice.
